Developmental dysplasia of the hip (DDH) describes a spectrum of abnormalities where the hip joint does not develop normally. With early diagnosis and the right treatment, most children achieve excellent long-term outcomes.

What is DDH?

DDH describes a spectrum of abnormalities where the femoral head (ball) may be loose in the socket, partially dislocated, or fully dislocated. It occurs in approximately 1–3 per 1000 live births.

Signs and Screening

• Newborn: hip instability on Barlow/Ortolani testing
• Infants: asymmetric skin folds, limited hip abduction, leg length difference
• Walking children: abnormal gait, Trendelenburg sign, waddling
• Hip ultrasound is the standard diagnostic tool under 6 months; X-ray after 6 months

Treatment Approach

Treatment depends on age at diagnosis and severity. Newborns and infants are typically treated with a Pavlik harness. Older infants may require closed or open reduction and casting. Dr Cundy holds an Advanced Graf Ultrasound qualification for DDH diagnosis in infants.
